Tseung Kwan O Hospital is investigating a medication incident in which a newborn boy was given an incorrect dose of Bacillus Calmette-Guerin vaccine.
The baby was admitted to the hospital's paediatric ward for feeding intolerance and vomiting at the end of August. His condition improved and he was discharged on September 4. He was administered the vaccine before he left the hospital.
In a routine drug check today, the hospital learned the vaccine administered to the baby was another product containing Bacillus Calmette-Guerin vaccine. The baby had been given about 10 times the normal dose.
According to literature reviews, the administered product would not harm the baby, and only a mild allergic reaction would be found on the injection site.
The baby was admitted to Queen Elizabeth Hospital for observation because of mild jaundice and is now stable. The hospital will monitor his condition and provide necessary care and treatment.
Investigations found the incident involved a human error. The hospital will bolster staff training and supervision, and take appropriate action according to the Hospital Authority's human resource policies for the staff concerned after a detailed investigation.
The hospital apologised to the baby's family and gave assurances that staff members will learn from this incident to avoid recurrence.
