Medication incident report to be finalised

The Special Investigation Panel studying a Prince of Wales Hospital medication incident will finalise a report in two to three weeks, putting forth recommendations to rectify deficiencies identified in chemotherapy's adminstration system.

 

The panel was set up after a 21-year-old patient, who had acute lymphoblastic leukaemia, was given a dose of Vincristine via the intrathecal route during her maintenance phase treatment at the hospital on June 15. The woman passed away on July 7.

 

The panel, headed by Canada Princess Margaret Hospital Medical Oncology Prof Ian Tannock, comprises Hong Kong Academy of Medicine President Grace Tang and Hospital Authority Quality & Safety Director Dr Leung Pak-yin. It is investigating circumstances surrounding the incident and making recommendations to prevent recurrence of similar incidents across the authority.

 

On Thursday, the panel visited the hospital's Ambulatory Cancer Care Centre where the medication incident took place. A total of 21 relevant people provided written information, with 11 attending an interview with the panel. The panel also met with the deceased patient's family yesterday.

 

The panel will table the report to the Hospital Authority Chief Executive.
